Succeeding with weight management in busy clinics by transforming care systems

Gap addressed:  Obesity is not effectively being addressed in many health care settings. This session will consists of a presentation and a workshop. The presentation will describe an approach to transforming care systems with several large Colorado clinical networks (which can also be applied to other geographic areas). The workshop will go into detail how we are working with clinics as part of our Wellbeing Campaign.

Learner's Objectives:  At the end of this activity, participants should be able to:

1) describe barriers to effective weight management in busy clinics

2) initiate effective weight management conversations

3) understand how to promote longterm behavior change from a systems change perspective
